When the care of an older relative has reached the point of being
too complex to manage, a geriatric care management program
provides you with peace-of-mind.
Introducing Jewish Family Services Family Connections
This new care management program is designed to ensure that
proper support services are in place to keep your older adult relative
as independent as possible. Beginning with a comprehensive in-
home evaluation, the JFS Care Management team will develop a
personalized plan of care, including recommendations for services
and available resources, as well as be available for ongoing care
management.
When its time to look outside the family for help, choose a partner
you can trust.
Family connections is the only geriatric care management program in the Charlotte
Jewish community. It is funded in part with a grant from BJH Foundation for
Senior Services and the Jerome Madans Fund and the Stan
Greenspon Charitable Fund.
