Thompson, RN, FSU Nurse Supervisor Students WITHOUT documentation of ALL required immunizations on file in the Student Health Services (Spaulding Infirmary) must FIRST report to the Spaulding Infirmary PRIOR to entering the Registration Arena. North Carolina State Law requires that all new students (and students who have not been enrolled at Fayetteville State University within the past academic year) present PHYSICIAN OR CLINIC-CERTIFIED PROOF OF REQUIRED IMMUNIZATIONS. The minimum required immunizations are; 1) Teiai\us-DipVnheria which musi have been received within the last ten years. 2) Polio (oral) - proof of three doses required if under age 18. 3) Rubeola (measles) - required if bom after December 31, 1956. Must have been received on or after first birthday. 4) Rubella (German Measles) - required if under age 50. Vaccine (after 6/9/69) or proof of immunity by blood lest (titer). 5) Tuberculin skin test - must be dated within one year prior to registration. THE STATE OF NORTH CAROLINA REQUIRES THAT STUDENTS WHO FAIL TO PRESENT PROOF OF THEIR IMMUNIZATIONS BE ADMINISTRATIVELY WITHDRAWN FROM THE UNIVERSITY. THIS ADMINISTRATIVE WITHDRAWAL CAN RESULT IN LOSS OF MONEY, GRADES, AND/OR CREDIT FOR ANY COURSES. INSURANCE WAIVER INFORMATION.... Some health care services are outside the scope of our operation. Any services NOT offered through the Student Health Services must be obtained by students at their own expense or by using the Medical Insurance Plan. The Medical Insurance Plan is a group accident and sickness insurance for students (includes pregnancy benefits). The premium is included in each undergraduate student’s semester bill. Undergraduate students who do NOT wish to be enrolled in the Student Insurance Plan MUST complete the insurance waiver form and present proof of personal insurance coverage at Student Health Services each semester during registration or no later than five working days aftei; the date of registration.
