Silicone gel breast implants leak much more often than previously believed, and one Japa nese study indicates a link, although inconclusive, between the gel and an immune system disorder, a government panel was told Tuesday. The panel of experts advising the Food and Drug Administration has three days to decide whether the implants now in the bodies of nearly a million American women are safe. "If we are comfortable that we can give sound advice to women who al ready have these implants, then, and only then, can we consider continued use of these devices," FDA Commis sioner David Kessler told the advisory panel. Such advice won t be easy in com ing. Women with leaky implants some times have no symptoms; routine mammograms would expose patients to relatively high doses of X-rays, and any kind of surgery carries risks. Manufacturers say the devices are safe, and women who have had the implants after breast cancer surgery say they need the devices to cope. Dr. Gordon Johnson, the FDA's di rector of health affairs, raised most of these points with the committee and ended his speech by wondering what ought to be said to the woman with implants but no symptoms of leakage, such as inflammation or a change in the shape of the breast. At the FDA's request, manufacturers and surgeons have stopped using the implants until the safety issue is settled. Lawsuits and consumer groups have Jell-0 alleged that gel leaking from the im plants is responsible for a host of health problems, including cancer and dam age to the body's immune system. The agency said there had been no conclu sive studies to demonstrate such links. Even in implants that don't break, silicone gel can "bleed" through the plastic coating enclosing the implant. "One of the things that concerns me most is that these implants may rupture, leak or bleed more frequently than we had originally thought," Kessler said. He said no firm rupture rate had been established. Later testimony from an FDA staff member, Tom Callahan, indicated the rate might be as high as 14 percent, although the industry 's figures are much lower than that. However, the company said it wouldn't make the documents public unless the FDA requested that. Last week, Dow Corning released more than 100 documents about im plants after the FDA insisted. Those documents showed that com pany officials had doubts about the safety of implants while reassuring the Vote public they were safe.
