JFS Family Connections
History and Purpose
Family Connections was
designed to meet the needs of eld
erly or disabled residents of
Greater Charlotte whose families
reside outside the area. The serv
ice also helps family members
who cannot monitor the well
being of their loved ones by aiding
them in advanced planning for the
handling of any emergencies
which may arise.
Staff
Family Connections is staffed
by a professional case manager,
Leah Gevirtz, experienced in
working with the elderly and dis
abled. The JFS case manager con
nects out-of-town family members
with the day-to-day care needs of
their older or disabled relative.
While respecting the independ
ence and dignity of the senior
adult. Family Connections strives
to relieve family members of the
fear and worry that distance can
create.
Testimonial
“JFS and Family Connections
has been a true life-saver for us.
Not being able to physically be
there for my father has been frus
trating. But knowing that JFS vis
its with him, monitors his diet and
sits in on doctor appointments has
offered me and my siblings a
peace-of-mind that we didn’t think
we’d experience at this stage of
his life.”
Family Connections Process and
Services Provided
The initial contact is typically
made by interested family mem
bers to discuss the personal needs
of their family member with the
Family Connections Case
Manager. A professionally trained
case manager completes an assess
ment of the individual’s strengths
and needs, psychological and
social functioning, physical limi
tations, and independent function
ing. Based on the initial assess
ment, the Family Connections
Case Manager provides the senior
adult and family with an individu

Temple Beth El’s Sisterhood

When asked why she was
drawn to JFS, Susan says that
while she has longed served this

thing she couldn’t pass up. “I see
JFS as a perfect fit for me because
it allows me to be part of this
amazing community-wide effort.”
