Evidence of Identity
Always Needed
' Nearly everyone in the
Lumberton and Laurinburg
area who applies for a Social
Security number remembers
to bring evidence of age and
? citizenship, but many forget
to bring evidence of identity,
Johnny R. Johnson, Social
Security manager in Lumber
ton, said recently.
Evidence of identity is
always requested before the
Social Security number can be
assigned, no matter how
young the persor. is, Johnson
said. This rule applies to
infants and small children as
well as adults.
A person applying for a
Social Security number usual
ly needs at least 2 pieces of
evidence: one to establish age
and citizenship and the other
to establish identity.
The best evidence of age
and citizenship is a public
birth certificate established
before a person's 5th birthday
which was issued by a U.S.
State or local government. Of
course, Johnson said, a for
eign born person needs the
appropriate immigration or
naturalization records as well.
Some people find it difficult
to find evidence of identity for
? x lti J
imams or young cnuarcn,
Johnson said. Examples of
documents that can be used
include ? accination certifi
cates; newspaper announce
ment; day care or nursery
school record; adoption re
cord; or doctor, clinic, or
hospital record.
Older children can use
school records, membership
in Boy Scouts or Girl Scouts or
other youth organization, re
port cards, ID card, or welfare
case record.
In fact, Johnson said, any
document that gives enough
identifying date to establish
proper identity can be used.
Parents who apply for a
Social Security number for a
child must also provide evi
dence of their own identity in
addition to the required evi
dence for the child.
Only original documents or
copies certified by the issuing
agency can be used. Uncer
tified or notarized photocopies
are not acceptable.
U.S. citizens applying for a
replacement Social Security
card to replace a lost card
need evidence of their iden
tity, while foreign-born per
sons need evidence of current
U.S. citizenship or lawful
alien status as well as evi
dence of identity.
People reporting name
changes need evidence of
identity under both the old
and new names before a new
card can be issued. |
Although people 18 or over
who are applying for a Social
Security number for the first
time must apply in person,
others may apply by mail.
Social Security will return all
original proofs submitted to
the applicant promptly. Do
cuments issued by the Immi
gration and Naturalization
Service should not be mailed.
More information about
applying for Social Security
numbers can be obtained at
the Lumberton Social Security
office, located at 322 E. 24th
St. The telephone number for
the Lumberton area is 738
8121 and the number for the
Laurinburg area is 276-5012.
A free leaflet can be obtained
which tells about other evi
dence that can be used.
