An additional statement is added to warn that the application is being made in connection with the receipt of Federal funds, that school officials may, for cause, verify the information in the application, and _that deliverate misrepresentation of information in the application, and that deliberate misrepresentation of information may subject the applicant to prosecution under applicable State and criminal statutes!. Under the provisions of the policy Rev. Francis^J7 O'Rourke, Administrator will review applications and determine eligibility. If a parent is dissatisfied with the ruling of the official, he may wish the discussion the decision with the determining official on an informal basis. If he wishes to make a formal appeal, he mav make a request either orallv or in writinc to Sr. i Edmund Marie, O.S.F. 1208 Hattie Avenue 722-2291, for a hearing to appeal the decision. The policy contains an outline of the hearing procedure it a family member becomes unemployed or if family size changes, the family ?-\ould contact the school to file a new application. Such changes may make the children of the family eligible for reduced price meals, or for additional benefits such as free meiris and mild if the family income falls at or below the levels shown above. In the operation of child feeding programs, no child will be discriminated against because of race, sex, color, or national origin., color, or national origin.
