Located near the hospital in the Professional Center at 804D North Lafayette Street, Care Solutions helps caregivers lo- cate resources in the communi- ty to make their care giving ef- forts easier. Care Solutions also performs other functions in the commu- nity. Not only do they help local caregivers, they assist those giving care long distance as well as those who do not have a family to help them when they need coordination of services. Care Solutions can make hospi- tal or home visits to meet the client, caregivers, and families, listen to their needs and situa- tions, research what is available to meet their needs, and offer options from which they may chose. They then develop a plan of care, coordinate the opinions and follow up with the family or caregiver to en- sure satisfaction of the services. As people interact with fami- ly members, and with those giving care, they recognize needs that Care Solutions can help with. Information on the services Care Solutions has available can be accessed by calling Anzie Horn R.N., or Jane Wright at 487-0968.
